Please help - survey for marketing project

So I'm taking this class on new product management this summer. For my mid-term/final exam, I have to do a full marketing plan on a new/failed product.

I'm doing Pepsi AM, which was an extra-caffeinated version of Pepsi that failed miserably in the late 80's.

For the early part of my project, I just need some really really basic survey data from folks.

So, if people wouldn't mind, please visit the following link and take/submit the attached survey:

Should take no more than 2-3 minutes.

It's a PDF, because I know Acrobat inside and out... I tried to make a web survey and it bombed. I'll probably have more involved surveys later, so if this is a huge hassle for a lot of people, I'll try my hardest to come up with a better way.

Depending on which version of Acrobat you have (mainly pro vs reader) the submit button will open your default e-mail client and send the file. Pro users can save the PDF and send it that way.
