who uses in ear monitor systems? Need advice

Since I only have one pair of ears, i want to invest in a good in ear monitor system that gives me good sound isolation right out of the box. I've never used IEM's before, so i need some input on them.

I'm sure those of you who are in bands know that everyone especially the guitar players have a tendency to keep turning up the volume, or the drummer keeps telling them to turn up the amp because they cant hear them (drummers always are loud anyways). So before you know it, everything is just too loud and then you can't hear the vocalist because everything else is just too loud.......so they try to turn up their volume, but they get feedback. In my band's situation, everything just gets too loud and washed out, and my ears are ringing after the rehearsal.

So i need to invest in a good sound isolating in ear monitor system in order to hear the whole band, my vocals, without killing my ears.

I'm looking at the shure PSM200 model right now, it seems like a good unit........anyone use this??
If you use a different one, please tell me the model and your review of it so i can compare.
I'll probably invest in higher quality ear buds, as i read that the stock ones aren't that great......but i would appreciate some advice or opinions on the earbuds too........I need ones that will truly isolate the sound and not fall out of my ears.

I also have some other questions:

Can you achieve the same results using a headphone amp, and using professional in-ear buds that isolate sound?

Can you use a powered mixer with an in-ear monitor system and/or a headphone amp, or does it have to be a standard mixer that is not "powered"?? (not sure if a powered mixer has outputs that are suitable for connecting to the in ear monitor system or even a headphone amp)
