HELP (2 humbucker, 5 way switch)

HI, i need to do a provisory wiring for 2 dimarzio humbucker and a standard 5 way switch, i need it simpler that the middle positions (2,3,4) don't need to work. the Guitar also have 1 vol and 1 tone.

Someone have the diagram???
