SD Jazz/JB

need a wiring diagram for SD Jazz/JB, 1 volume, 1 tone and a 5 way switch,
the 5 way switch is whats confusing me the most, any pics or links will help

my S470 has the same setup. I put in a SH-13 last night. the red and white wires go together and soldered to the switch. the black wire gets soldered to the switch as well. the green and bare go to the back of the pot.

the switch i had is a straight 8, rather the normal 4 on each side.
