Question Locking nut wierdness

hey guys, I took my locking nut off on my RG520 around 3 weeks ago while doing a thorough cleaning of it. Well right now my problem since i put it back on is, the bass-side screw is further in, you can't really visually tell, but the treble side one is sticking out more just enough for it to be annoying. Each of the screws had washer under them, but the base side washer had kind of a star shape hole on the inside, instead of just round, which I though was weird.

TL;DR.

Did I screw up my nut, is this normal? the nut also has a bass side shim (in addition to 2 full shims). Tell me what might be up with it.

The star washer is a locking washer--Rich has them...

The flat washer goes against the wood, the star washer goes between that and the bolts head. Each side should have two washers.

thats strange. there was only one on each when I took it of. The star on on (bass), the normal on the other .

Would it be dangerous to leave it as it is?

Somebody lost two of the washers and then split them up. I don't think it would be dangerous, but I'd get some replacements.
