Poor Man's Input Jack Replacement

I opened up a guitar that just arrived to rewire its tone pot as someone had mixed the wires up. Then I saw that someone had replaced the output jack as well. With a 1/4 inch to 1/8 inch adapter. The bottom one is the one from the guitar if you couldn't guess. Its rubber wasn't ripped up until I got to trying to get the thing out of the guitar. Some people.
