I have small hands, and a 43mm nut is uncomfortable, I have a usa custom with a 42mm that fit perefectly. What all would be involved in taking a 43mm nut width neck and cutting it down to 42mm. I'm thinking pull the first 5 frets or so, draw my lines, and carv her down, tapering it to the 5th fret and then refret the top ones. Am I oversimplifying?

You don't have to pull the frets. You can do it all with a belt sander if you have one, and/or sanding blocks. Then finish with a file and dress the fret ends as if it were a refret. But you wouldn't take it just to the 5th fret, you should blend it all the way to where it joins the body. It won't be much.
